Online Forms” below.
· You will then be prompted to create
a Parent Account or Continue as a Guest to complete the online
forms.

· In order to complete the forms, you may be asked to provide your student’s ID number and/or Date of Birth along with other demographic information.

Sports Physical Information:
The CC Athletic Training staff, along with Dr. Yocum, are
planning to host physicals at the end of May and July for this coming sports
season. Signups for physicals will be done through Microsoft Booking. Follow
the link below to the Booking Page to set up a physical appointment. There are
instructions outlined below that are also on the Booking Page. Please select
“Read More” under PIAA Physical Sign Ups once on the Booking Page.
You must be logged into your student’s Microsoft account to
access the Booking Page.
The cost for the physicals is $15 cash or check accepted at
the door.
Booking Page Instructions: Use the calendar to sign up
for an open time slot for a PIAA athletic physical. Click the arrows to advance
the calendar to May 29th or July 31st. There will be 4
open appointments per time slot and then the time will no longer be available
for selection. Please enter the name of the athlete receiving the physical in
the details section below the calendar. Add any email addresses to receive
appointment notifications and reminders. Make sure you are creating an account
on RankOne or logging in to fill out the online forms BEFORE your physical
date. If you have any questions, please reach out to the athletic training
staff.

Just like previous seasons, ALL physical
forms must be completed in RankOne. All athletes scheduling a physical at
Cedar Crest must complete all forms 24 hours prior to their appointment. Physical
forms for the 2025-2026 sports season can be accepted in RankOne starting May 1st. Please refer to the
Cedar Crest Athletics website for all information and instructions for RankOne.

typically offer summer physical options. We have not received any information
from either of these health care providers, but as soon as we receive the
