Sports Physical Information:
Athletes should have all the necessary forms completed
in Rankone by Friday 11/7/2025 at 11:59 pm, to be eligible for
winter sports that start on Friday 11/14/2025. Athletes who complete paperwork
after 11/7/2025 will completed in the order received with no guarantee they
will be cleared for the first day of winter sports.
Athletes who completed a physical prior to October 3,
2025, will need a section 7 completed to be eligible for winter sports on
11/14/2025.
If a fall athlete had an injury and could not complete the season due to the injury, they will need to have their family physician complete a section 8or have one completed by our team physician. We hope to have those athletes who need a section 8 attend the physicals being held on November 6 2025. More information to come.
CLSD is offering a Winter/ Spring physical on Nov. 6
between 3:30 and 5:30 . The location of the physicals will be in the high
school athletic training facility and athletes will enter from and then travel down the flag hallway to the pool hallway where they will be
able to confirm appointment time and pay the $15.00/ person for the physical.
Payment is cash only or check. Checks are made to the Cornwall-Lebanon School

Just like previous seasons, ALL physical
forms must be completed in RankOne. All athletes scheduling a physical at
Cedar Crest must complete all forms 24 hours prior to their appointment.
