Please note that athletes should complete Sections 1-5 and the risk of injury form on RankOne before their physical. They should then take the completed forms with them or have them available through the RankOne app to show the doctor at their appointment. Athletes SHOULD NOT follow the QR code to the PIAA forms that OAL provides. We will not accept those forms as we only accept the forms through RankOne.
