SPORTS PHYSICALS AT NO COST!!!
Lebanon Family Health Services is offering Free Sports Physicals (times and dates below).
Walk-Ins are welcome OR- Appointments can be made by calling 717-273-6741.
The PIAA Section 6 Physical Form must be completed and taken along to the physical.
LFHS is located at 615 Cumberland Street in Lebanon

Wednesday, July 31: 3pm-5pm
